Job Title: Personal Supports Case Manager

Department: Personal Supports

Reports To: Deputy Director

FLSA Status: Non-Exempt

Approx hrs/wk: 20

Summary

Plans, coordinates, and directs the Personal Supports Program for people with developmental disabilities, implements competent records management, oversees implementation of staffing, and ensures services are provided according to Agency policies and procedures.

Essential Duties and Responsibilities include the following. Other duties may be assigned.

Oversees the management of all DDA-funded individuals involved in the ISS and Personal Supports Program. Observes and evaluates staff supports; advocates for needed services; obtains additional resources as needed.

Reviews and approves attendance, invoices, and EVV records to ensure accuracy and timeliness of billing

Attends meetings for Person Centered Plans and completes PCP packets; forwards PCP packets to appropriate people on the team. Files all PCP documents according to Agency standards. Ensures that staff are trained on the PCP and are implementing the Person-Centered Plan accordingly.

Participates in all ongoing in-service training and maintains all required DDA and agency training requirements including certifications and re-certifications in a timely manner.

Meets face-to-face with people receiving ISS/Personal Supports on a quarterly basis or more often if needed.

Supervises, trains, and evaluates all staff providing direct support services in accordance with each individual’s Person-Centered Plan.

Documents all services on a Care Note as a record of case management services provided.

Reviews Monthly Summaries/Progress Notes/CareTracker logs submitted by staff and checks for accuracy and completeness. Ensures that all required documentation is filed according to Agency standards.

Organizes and maintains the records system for people supported in ISS/Personal Supports.

Represents Creative Options at professional and community meetings.

Maintains and supports Creative Options “Vision,” “Mission,” and “Motto” statements.

Performs CPR and First Aid as needed.

Ensures individual rights are protected and that regulations related to safety and health are maintained in accordance with agency and licensing standards.

Maintains ethical and competent professional relationships and the highest quality of delivery of services.

Supervisory Responsibilities- Carries out supervisory responsibilities in accordance with the organization’s policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.

Education and/or Experience- 4-year Degree and 5 years’ experience working directly in a related position or equivalent combination of education and experience.

Language Skills-Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, procedure manuals, and DDA, DHR, and MHA service funding plans. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the public.

Mathematical Skills-Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.

Reasoning Ability-Ability to apply common sense understanding to carry out detailed but uninvolved written or oral instructions. Ability to deal with problems involving a few concrete variables in standardized situations.

Computer Skills-To perform this job successfully, an individual should have knowledge of Database software and Word Processing software.

Certificates, Licenses, Registrations-Valid Maryland driver’s license with a good driving record.

Safety and Training Requirements-Must successfully complete all agency and DDA training requirements within first 90 days of employment and maintain required training re-certifications.

Other Qualifications-Knowledge of DDA service delivery system and budget system. Knowledge of generic resources and other stated relates services.

The employee must regularly lift and/or move up to 10 pounds, frequently lift and/or move up to 25 pounds and occasionally medium work that includes lifting and/or moving up to 50 pounds. Communicating verbally and/or electronically to exchange information.
