Job Summary: Familyway Home Care is seeking a compassionate and experienced Per Diem Case Manager / Registered Nurse (RN) to join our team. The role involves coordinating patient care, ensuring compliance with regulations, and overseeing home health aides. The Case Manager/RN will serve as the primary contact for clients and families, developing and implementing personalized care plans.
Key Responsibilities: Patient Care Coordination:
Assess, develop, and oversee individualized care plans for home care clients.
Conduct assessments to evaluate health status and needs.
Monitor and ensure quality of care provided by aides.
Educate patients and families on health conditions, medications, and self-care.
Compliance & Documentation:
Maintain accurate clinical documentation per guidelines.
Ensure services comply with Medicaid, Medicare, insurance, and Joint Commission standards.
Perform regular home visits to monitor progress and adherence.
Supervision & Staff Coordination:
Train, supervise, and evaluate home health aides.
Collaborate with healthcare professionals to coordinate care.
Act as liaison among agency, clients, families, and providers.
Quality Improvement & Risk Management:
Address patient safety concerns.
Participate in quality assurance and case conferences.
Assist with audits and inspections.
Qualifications & Requirements: Active RN license in New Jersey.
At least 2 years of nursing experience, preferably in home health or case management.
Strong knowledge of healthcare regulations and insurance guidelines.
Experience in care planning, assessment, and interdisciplinary work.
Excellent communication and organizational skills.
Ability to work independently and manage multiple priorities.
Proficiency with electronic medical records (EMR).
Preferred Qualifications: Experience with geriatrics, post-surgical, or chronic disease management.
Knowledge of state and federal home health regulations.
BLS/CPR certification.
