Summary
The RN Case Manager coordinates the care and service of patient populations from admission through discharge. The RN Case Manager collaborates with interdisciplinary teams through effective case management planning to achieve clinical and financial goals.

Responsibilities

Actively coordinates patient care, monitors length of stay, leads rounds, and works to meet clinical targets.

Collaborates with Social Worker to identify the need for team and family conferences and facilitates care transitions.

Monitors patient and family satisfaction, responding to queries and complaints.

Performs chart reviews to identify and resolve barriers to timely discharge.

Conducts in‑depth case management assessments and develops comprehensive discharge plans.

Consults with physicians and multidisciplinary teams about patient status and care appropriateness.

Additional Job Information
This role requires critical thinking, effective communication, decisive judgment, and positive relationship building. Minimum one year of case management experience in a hospital setting or five years in healthcare is required.
