Hybrid Pediatric RN Care Manager
Join HealthCare Support in a hybrid role helping patients and families get the support they need. This position offers a work‑life balance with no weekends, evenings, or holidays.
Location
Boca Raton, Boynton Beach, or West Palm Beach, FL.
Pay
$55,100 - $99,000 a year.
Shift
Monday – Friday, 8 a.m. – 5 p.m. (no on call, weekends, evenings, or holidays).
Daily Responsibilities
Coordinate and manage comprehensive care plans for medically fragile children (ages 0–21), including post‑hospitalization assessments, follow‑ups, and connections to community resources to improve outcomes and reduce readmissions.
Collaborate with families, providers, and specialists to address medical and social needs, ensure continuity of care, and complete documentation remotely.
Required Qualifications
Active, unrestricted Florida state Registered Nurse (RN) license.
2+ years of recent pediatric experience as an RN.
