The Patient Care Manager (PCM) combines AHF's mission with psychosocial case management, leveraging extensive social work experience, understanding social determinants affecting the HIV population, and knowledge of the Ryan White Care Act Program within their area. Working closely with providers and the healthcare team at the Westside HCC Healthcare Center, the PCM will assess patient needs, develop service and care plans, and support patients in achieving their health and social goals. Responsibilities include providing education on eligibility for programs like Ryan White, conducting health risk and needs assessments, and facilitating referrals to community resources. The role requires understanding mental health, housing, food security, and other basic needs, and building relationships with community agencies to support patient care. The PCM’s interventions will enhance patient understanding of their health and social determinants, fostering better health outcomes.
Utilizing your case management expertise, you will meet clients where they are, identify strengths and opportunities, and assist in setting realistic goals. Your collaborative approach will embody AHF's mission and support the safety net for uninsured HIV/AIDS patients, directly impacting patient well-being and contract deliverables. ***HYBRID WORK ARRANGEMENT AVAILABLE AFTER 90 DAYS*** - 1 day per week, no Mondays or Fridays.
