Patient Care Coordinator - Community & Outpatient Services

This position requires seeing patients in Gainesville twice a week with administrative offices in Ocala. Only apply if you are able to travel to Gainesville twice a week.

Are you passionate about helping people and making a difference in your community? We are seeking a Patient Care Coordinator to join our outpatient medical services team. This role is ideal for someone organized, compassionate, and adaptable — ready to support patients in clinics, community settings, and secure facilities in Florida.

What You’ll Do

• See patients in Gainesville and in Ocala Florida
• Perform basic health screenings and collect blood and urine specimen collection (training available). Phlebotomy experience a must
• Assist with intake, scheduling, follow-ups, and EMR documentation.
• Participate in seeing patients as part of a team at community sites and correctional facilities (requires local and regional travel
• Collaborate with clinical staff to ensure continuity of care.

What We’re Looking For

• 1+ years of experience in healthcare, medical assistant, social services, or a related field.
• Strong communication and organizational skills.
• Comfortable working with diverse populations.
• Ability and willingness to travel throughout Florida and occasional Saturdays as needed.
• comfortable using EMR/EHR systems (or eager to learn).

Bonus Points (Preferred Skills)

• Phlebotomy or specimen collection experience.
• Knowledge of HIV/STD prevention, counseling, or risk assessment.
• Travel flexibility
