Patient Care Coordinator

The Patient Care Coordinator (PCC) collaborates and ensures professional nursing practice in a clinical setting by: applying the nursing process; assessing the patient's clinical condition while prescribing appropriate nursing actions to meet the needs of the patient; assuring quality patient care utilizing standards of practice established by the facility; providing educational opportunities with patients and other significant individuals; and communicating effectively using both verbal and written abilities. The PCC works closely with the clinical manager in planning, coordinating and evaluating the activities of the patient care unit. Participates as a leader in the customer service program, champion for rounding, service recovery, and is aware of employee and customer satisfaction scores. The PCC oversees completion of unit duties during shift, attends unit and shift supervisor meetings and may provide direct patient care dependent upon staffing needs of the unit.

  • Quality of Nursing Evaluates and monitors the quality of nursing care administered by the nursing staff on the unit.
  • Staffing Identifies oncoming shift's staffing needs and initiates action that assists in procuring adequate levels of staffing within their control.
  • Rounding Performs regular rounding for outcomes on patients and staff.
  • Shift Report Prepares and delivers the end of shift reports to oncoming shift Charge Nurse.
  • Patient Assessment Assesses the patient's clinical condition accurately while prescribing appropriate nursing actions to meet the needs of the patient.
  • Ingenuity Displays ingenuity, self-reliance, resourcefulness, assertiveness, ambition, and anticipation of what needs to be done and follows through with assignments. Takes needed action without direct instructions. Participates in committees, employee events, and activities.
  • Patient Needs Data Collection Prioritizes data collection activities based on the patient's immediate condition or anticipated needs.
  • Compliance Remains current and shows compliance regarding new developments and technology in areas of responsibility, including documentation, medication administration, monitoring systems, etc.
  • Knowledge of Standards Utilizes knowledge of federal, state and accreditation standards and regulations in the unit setting.
  • Complaint Management Performs timely and effective service recovery to address patient/family complaints and grievances.
  • People Management Activities Participates in people management activities for direct team members such as conducting performance evaluations, disciplinary actions, and interviews.

Qualifications

  • Required- Registered Nurse (RN) license to practice in the state or as listed in the Nurse Licensure Compact (NLC)
  • Required: Associate's Degree or
  • Required: Bachelor's Degree- For external candidates, required upon hire
  • Required: Bachelor's Degree- For internal candidates, obtain BSN within 3 years of promotion to position.
  • Required: 3 years Nursing/Patient Care experience
  • Preferred: 1 year Area of practice or related specialty
  • Required: Basic Life Support Program (BLS) - American Heart Association
  • Required if required by unit: Unit Specific Certifications - Franciscan Alliance
  • Required if required by unit: Advanced Cardiovascular Life Support (ACLS) - American Heart Association

Travel is required.
