About This Role
The Participant Care Coordinator is the welcoming point of contact for families exploring services at Stepping Stone. You will guide families through intake and enrollment, provide tours, manage waitlists, attend service/resource fairs, and educate on HCBS waivers to ensure participants are matched with the right supports. Youll set a positive, professional tone while ensuring compliance, and will become a Certified IDD Investigator (training provided) to conduct internal reviews and investigations as needed. This role requires exceptional organizational and interpersonal skills to bridge communication across teams, families, and service providers.
Operational Excellence
You will oversee key operational functions, including managing ongoing documentation, responding to interested families, and conducting home inspections. Additionally, youll manage releases, create and maintain participant profiles, and conduct internal investigations after becoming certified. Your attention to detail will ensure all processes are carried out efficiently and meet compliance standards.
Team and Participant Support
As the primary point of contact for families, youll guide them through the intake process, complete participant profiles, and recommend services based on individual needs and funding sources. Youll coordinate with directors and service lanes to ensure participants are matched with the right services, fostering a welcoming and supportive environment. Your communication and follow-up will help families feel informed and confident while building strong relationships across the organization.
Expected Outcomes:
Within 15 Days
- Build rapport with team members, participants, and families, understanding their needs and expectations.
- Begin operational and role-specific training, including navigating CO Medicaid services, intake processes, and documentation standards.
Within 30 Days
- Independently perform participant intakes, maintain waitlists, and conduct HUD inspections with support.
- Begin contributing to outcome-based metrics, including manager/director feedback and process accuracy.
Within 45 Days
- Demonstrate mastery of the role.
- Show results through smooth operations, reduced documentation errors, and high satisfaction among participants, families, and staff.
Requirements:
- Minimum of a high school diploma
- 1+ years in case management, human services, admissions, care coordination, customer service, or similar field (IDD-specific experience preferred)
- Proactive approach, excellent follow-through, and ability to accurately handle legal/confidential docs
- General understanding of IDD services and Colorado HCBS waivers, or willingness to learn
- Current certifications in CPR/AED, First Aid, QMAP (training provided if needed)
- Ability to work effectively with a collaborative team
- Experience working with at-risk populations (disabled, elderly, children)
- A valid drivers license and current auto insurance for vehicle used on company time (must be at last 21 years old and have held a drivers license for a minimum of three (3) years)
- Successful completion of CAPS check, criminal background check, and Motor Vehicle Record check (no Type A violations or more than two (2) Type B violations on your driving record within the past three (3) years)
- Warm, clear, professional, and timely communication in all interactions
- Willingness to learn SharePoint, DocuSign, Quickbase and have strong email management and general tech abilities
- A professional presence that is
calm, organized, groundedespecially during sensitive conversations or investigations
Schedule:
- Full-time, 3240 hours/week, plus mandatory staff meetings
- Flexibility required for occasional evenings, weekends, or urgent investigations
- Work primarily at the office (9032 W. Ken Caryl Ave., A-1, Littleton, CO 80128), with fieldwork as needed (Note: Were moving to 5894 S. Zang St., Unit 1A, Littleton, CO 80127 by the end of 2025.)
- Typical hours: 9:00 a.m. 5:00 p.m.
