Part‐Time RN Care Manager ‐ Home Care

Who You Are

• You have a current CA RN license
• If no current RN experience, You have home health experience as an LVN – you understand home care settings, patient/family interaction, documentation in the home environment, and the flexibility/adaptability it takes.
• You’re organized, compassionate, and able to coordinate care plans, follow‐ups, and communication with patients/families and other care providers.
• You have strong communication skills—verbal, written—and are comfortable handling both clinical visits and follow‐up/oversight tasks.

What You’ll Do

• Manage a caseload of clients in their homes: conduct assessments, plan of care, periodic visits, and coordinate with LVNs/Home Care Aides/therapists/physicians.
• Oversee care plan progress: monitor outcomes, adjust plan as needed
• Provide education to patients & families about health needs, care instructions, safety, etc.
• Document accurately and timely: charting, reports, communicating changes, ensuring compliance with CA home care standards.
• Travel locally to client homes (mileage reimbursed / travel time accounted for).

Requirements

• Current and unrestricted RN license in California
• If no current RN experience, a minimum of 3 years of LVN Home Health experience is required
• Infusion experience a bonus but not required
• Experience in Pediatrics a PLUS
• Proof of eligibility to work in the US
• Reliable car / auto insurance
• Clean background and criminal record
• Current CPR/BLS
• Recent Physical, TB/Chest X-ray on record
