We are seeking a Part‑Time Hospice Registered Nurse Case Manager to serve patients in Hunterdon and Warren Counties, NJ. The role is carried out remotely with patients’ care delivered in their homes.

About the Role
As part of the interdisciplinary team, the RN Case Manager works under the direction of the Director of Nursing, the attending physician, and other team members to evaluate, plan, and deliver skilled nursing care that maximizes patient comfort and support for families and caregivers.

Assigned a patient caseload by the Patient Care Coordinator.

Assume primary responsibility for a patient caseload; assess, plan, implement, and evaluate the nursing process.

Initiate communication with attending physicians, hospice staff, and other agencies to coordinate optimal care.

Maintain regular communication with the attending physician about patient and family needs.

Seek input from other team members regarding the patient’s plan of care.

Provide accurate and timely documentation in electronic medical records.

Participate in Interdisciplinary Group meetings and patient care conferences.

Coordinate physical care by teaching caregivers or providing direct care.

Meet regularly with the hospice nursing staff to review case issues.

Document and supervise the Nursing Assessment and Care (NAC) as required.

Inform the Patient Care Coordinator of unusual or potentially problematic patient/family issues.

Assume the role of associate nurse during on‑call time or as requested by the Coordinator.

Provide support at the time of death and during bereavement.

Participate in hospice orientation and in‑service training programs.

Engage in agency and community programs as required.

Maintain familiarity with agency policies and state/federal hospice regulations.

Accept other assignments as appropriate.

Qualifications

Graduate of an accredited School of Nursing.

Licensed RN in NJ; compliance with NJSA 45:11-23 and NJAC 13:37.

Minimum one year RN experience; hospice experience preferred.

Excellent assessment and problem‑solving skills.

Strong organization, time‑management, and case‑management knowledge.

Valid driver’s license and insured vehicle in good working order.

Excellent verbal and written communication skills.
