Under the supervision of the Associate Director, Supportive Housing, the Case Manager is responsible for oversight of resident services programming at properties that serve high‑needs individuals, families, seniors, and residents with special needs.

Essential Duties and Responsibilities

Deliver onsite programs, ensuring implementation at assigned locations to meet internal standards and regulatory expectations.

Conduct outreach via door‑to‑door, phone, email, and other methods to inform residents of available services.

Provide crisis intervention and follow‑up, including mandatory reporting for abuse.

Support case‑management services for residents with functional impairment, facilitating independent‑living skills.

Offer individual and group services, and support community‑building activities at assigned properties.

Collaborate with residents to foster independent living and personal/professional goals.

Advocate for residents and educate them on program options.

Maintain HIPAA policies and procedures.

Partnership Management

Engage with property‑level partners to support partnership goals and criteria.

Collaborate with local community agencies near assigned property.

Work with Eden Property Management, Resident Services staff, and community partners to resolve property concerns or neighbor disputes.

Customer Service

Support property‑level needs assessments and follow‑up (e.g., community meetings).

Participate in developing responsive property service plans.

Communicate and execute actions based on assessed needs.

Provide exemplary customer service and responsiveness to residents.

Data‑Driven Decision Making

Maintain accurate property‑level records and files.

Ensure timely reporting in accordance with Services policies and training.

Effective Team Building

Participate in staff and team meetings, trainings, group outings, and site events.

Inform supervisor of activities and incidents promptly.

Cultivate collaborative relationships with Property Management peers.

Perform other duties as assigned.

Qualifications
Education and Experience

M.S.W., M.A. in psychology, counseling, or related field and 1 year of full‑time experience with senior/special‑needs populations, or B.A./B.S. and 3 years full‑time experience, or High School Diploma/GED plus 5 years of relevant experience.

Experience working with homeless populations and diverse groups.

Experience using assessment tools and interpreting data.

Experience supporting program implementation and coordination.

Experience collaborating with local service providers.

Knowledge, Skills and Abilities

Excellent communication skills, including across language barriers; proficiency in additional languages may be required.

Strong judgment, discretion, and problem‑solving.

Excellent relationship‑building, listening, and conflict‑resolution.

Attention to detail, organizational skills, and ability to work independently.

Commitment to learning and staying current in the field.

Proficient in Outlook, OneDrive, Teams, Zoom, Salesforce, Microsoft Office, and related tools.

Physical Requirements

Primarily desk‑based; occasional standing, walking, and lifting up to 15 pounds.
