Benefits and Schedule
This is a part-time role. Schedule: two days per week 4:30 PM to 8:30 PM; Fridays 11:30 AM to 8:30 PM; Weekends 7:15 AM to 1:15 PM. Responsibilities
The Assistant Case Manager provides case management support for a Day Reporting Center, checks in participants, and assists Case Managers with case coordination, documentation, and intake processes. Specific duties include:
Greet participants entering the facility.
May complete drug screens according to the assigned random schedule for each participant check-in.
May perform breath analysis on participants and provide written or computerized results.
Complete initial intake with participants, including paperwork, database input, and assignment of groups and services.
Provide general office administration and customer service to participants, customers, and the public.
Assist Case Managers with case coordination, communication with appropriate agencies, and preparation/distribution of reports.
Assist in managing a participant caseload using Principles of Effective Intervention (What Works).
Conduct risk/need assessments and create individual treatment plans for assigned participants.
Meet regularly with participants and document progress in the Company database.
Facilitate evidence-based CBT programs and other life skills programs in groups weekly; document attendance, participation, and progress.
Develop and monitor participant progress relative to CMP across program phases.
File, prepare charts, and perform data entry as assigned.
Open/close office procedures; may install/remove electronic monitoring equipment and forward documentation to the appropriate agencies and the monitoring center; may clean EHM equipment.
Perform other duties as assigned.
Qualifications
Minimum Requirements
High school diploma or equivalent required; college coursework in Human Services or business preferred.
Minimum of six (6) months experience in Human Services with direct participant services preferred.
Minimum of six (6) months experience working with minority populations preferred.
Must be twenty-one (21) years of age with at least five (5) years driving experience if required to transport participants; valid state driver’s license and five (5) year safe driving record if required.
Ability to safely operate a motor vehicle if required to transport participants.
Good customer service and communication skills; strong organizational skills and attention to detail.
Solid computer skills with MS Word and Excel; basic database and math skills.
Bi-lingual English/Spanish may be required.
Ability to work with the software typically used by the department.
