Overview

We are seeking a compassionate and experienced Registered Nurse (RN) to join our Advanced Illness Management (AIM) Palliative Care Team . This is a field-based position that provides in-person andtelephonic care coordination, education, and assessments to patients with serious or life-limiting illnesses.

The AIM Palliative RN will collaborate with an interdisciplinary team including Nurse Practitioners, Social Workers, and Primary Care Providers. This role ensures high-quality, patient-centered care and supports comfort, dignity, and quality of life for patients in private homes, assisted living communities, and skilled nursing facilities.

Key Responsibilities:

+ Conduct in-home and telephonic nursing assessments and screenings for palliative care patients

+ Complete risk stratification assessments and determine frequency of visits based on patient needs

+ Deliver patient and family education on disease progression, medications, nutrition, and end-of-life care

+ Collaborate with NPs and PCPs to identify changes in patient condition and escalate care appropriately

+ Coordinate care transitions and facilitate hospice referrals when needed

+ Accurately document clinical findings, assessments, care plans, and education in the Electronic Medical Record (EMR)

+ Support the development and regular updates of individualized Palliative Care Plans

+ Communicate lab results, order DME, and manage prescriptions in accordance with state RN scope of practice

+ Participate in rotating on-call coverage for evenings and weekends

+ Foster strong communication and collaboration across the care team, including social work and case management

+ Ensure adherence to all HIPAA, compliance, ethical, and professional standards

About You

Required Qualifications:

+ Active and unrestricted Registered Nurse (RN) license with no board actions

+ Minimum 2 years of clinical RN experience, preferably in skilled nursing, home health, palliative care, hospice, geriatrics, or oncology

+ Demonstrated ability to provide quality care in high-empathy environments

+ Proficiency in Microsoft Outlook, Word, Excel, and electronic medical records

+ Exceptional communication, time management, and critical thinking skills

+ Keen observation skills with the ability to identify changes in condition and escalate appropriately

+ Comfortable working independently in the field and within interdisciplinary care models

+ Valid driver's license and reliable transportation (field-based role)

Preferred Experience:

+ Previous experience in palliative care, hospice, or oncology nursing

+ Familiarity with risk stratification, symptom management, and home-based care planning

+ Experience working in or withassisted living and skilled nursing facilities
