Qualifications
Graduated from an approved school of professional nursing
One year experience as a Nurse Practitioner preferred
Licensed to practice as a Nurse Practitioner in the state of operation
Active DEA license in the state where care is provided
Valid CPR certification while employed in a clinical role (preferred)
Licensed driver with ability to travel to all business locations
Compliance with all state regulations governing palliative care services
Responsibilities
Develop and implement clinical plans of care for patients with chronic and terminal illnesses
Perform patient assessment, history and physical; provide recommendations for aggressive symptom management
Monitor patient progress and update the Medical Director, team members, and attending physicians; consult on discharge planning and patient/family education
Maintain communication with supervisory and attending physicians for routine review of patient records and care plans
Coordinate care with other Palliative Care team members and agencies to optimize resource use for patients and families
Provide support at the time of death as needed
Prescribe medication within state and Board of Nursing guidelines
Participate in continuous care for hospice‑appropriate patients and ongoing education programs
