Overview
PATH (People Assisting The Homeless) has pioneered bold and effective approaches to assist people experiencing homelessness. Since 1984 PATH operates services throughout California, connecting clients to a continuum of homelessness prevention, street outreach, employment preparation and placement assistance, individualized case management, supportive services, interim housing, and permanent supportive housing.
What You Do
The Outreach Case Manager helps clients break the cycle of homelessness by using a "whatever it takes" approach. This role supports clients through all phases of homelessness, including on the street, interim housing, transitional housing, and permanent housing. You will assist clients to access and maintain needed services among healthcare, social services, interpersonal support systems, and navigating a coordinated entry system (CES). You will provide individualized supportive services by helping each client develop a plan to address strengths, barriers, support health needs, increase income, and find and maintain housing.
Position Responsibilities
Conduct intake and individualized needs assessment for all participants and collaborate to develop an Individualized Service Plan (ISP) that addresses housing goals, income goals, and other needs (medical, mental health, substance use, financial resources, vocational, and social support).
Provide referrals, linkages, information, and support to resources that help participants achieve ISP goals.
Evaluate strengths and challenges to address short-term and long-term goals; conduct a 90-day review with the participant after initial intake.
Maintain documentation standards per program and PATH policies; timely case file notes and a complete, accurate participant file with all required documents.
Maintain confidentiality of participant files per HIPAA and applicable guidelines.
Develop therapeutic and professional relationships with clients and partners to enhance engagement and housing outcomes.
Provide crisis intervention and risk assessment to increase safety and support clients in solving problems and coordinating their own care, including services for individuals with chronic trauma.
Utilize motivational interviewing and trauma-informed care in service delivery.
Maintain current knowledge of community resources and use them to provide wrap-around services.

Preferred Qualifications
Two years of experience in healthcare, case management, homelessness services, or work with vulnerable populations or related field.
Minimum Qualifications
All levels of education and experience are welcome.
Mandatory Requirements
Have employment eligibility verification.
Have or be willing to obtain CPR/First Aid training.
Complete Tuberculosis Test, Background Screening, and Drug Test as a condition of hire.

We are a California employer. Regular work must be performed within the state. Approval for work outside the office does not imply outside-California work. PATH does not offer fully remote positions. Some roles may include hybrid sites, but many positions are on-site. Schedules are determined by role responsibilities and operational requirements and may be adjusted.
