The Outreach Case Manager will conduct outreach to a caseload of high-priority unsheltered people experiencing homelessness, administer VI-SPDAT assessments, help individuals collect identification documents to become document ready, and facilitate the housing process by acting as an initial liaison between the targeted individual and housing provider in various settings.
Duties And Responsibilities Provide outreach, case management, and housing navigation services in any setting necessary, including streets, parks, shelters, hospitals, jails, and encampments.
Provide light-touch and intensive outreach services as appropriate.
Provide referrals to medical, mental health, and substance abuse services as needed.
Assist targeted people with applying for identification documents, entitlements, and other benefits.
Build rapport and utilize motivational interviewing strategies to support vulnerable people through stages of change and accepting housing offers.
Access the Homeless Management Information System (HMIS) to input VI-SPDAT assessments and upload identification documents.
Maintain records and compile reports while ensuring client confidentiality.
Provide housing match services to facilitate successful housing placement.
Attend trainings as assigned.
Perform other duties as assigned.
Requirements Knowledge, Skills, and Abilities:
Understanding of systemic causes of homelessness.
Experience working with diverse populations.
Familiarity with homeless services and housing in Washington, DC.
Spanish proficiency preferred.
Computer skills, database input, and documentation skills.
Strong communication and boundary-setting skills.
Ability to work independently and collaboratively.
Ability to navigate DC via public transportation and on foot.
Ability to work in various settings, including outdoors.
Physical ability to work standing or walking up to 8 hours, lift up to 20 lbs.
Effective communication skills in English.
Experience:
At least 2 years in homelessness services, mental health, substance abuse, or case management.
Bachelor’s degree or relevant certifications preferred.
Physical and Mental Demands:
Ability to perform physical tasks and communicate effectively.
Travel Demands:
Occasional local travel with appropriate compensation.
