Outpatient Community Health Care Coordinator

The position supports care management and coordination activities in an outpatient setting. Partners with healthcare team to maximize patient engagement and patient outcomes. Identifies patients overdue for visits, labs, or referrals and initiates patient supportive services to reduce negative impact on patients' health and promote efficacy of clinic resources.

Schedule: 5 hours per day x five days

In addition to bringing humankindness to the workplace each day, qualified candidates will need the following:

  • High School Diploma/G.E.D. required
  • Minimum 3 years of experience in an outpatient medical practice
  • Proficient computer skills data entry, retrieval, and report generation, excel and virtual meetings
  • Certification as a Certified Medical Assistant or Certified Clinical Medical Assistant Required, or LPN Required Diploma from an accredited medical assistant's program and externship completed (if applicable) Current Colorado nursing license / current nursing license from a participating state in National Licensure Compact (if applicable)

Medium Work - exert/lift up to 50 lbs. force occasionally, and/or up to 20 lbs. frequently, and/or up to 10 lbs. constantly

Penrose Hospital serves as the anchor hospital of Penrose-St. Francis Health Services with 300-licensed beds and the ability to provide tertiary (high tech) medical-surgical services with an emphasis on elective and outpatient care.
