YOUR ROLE
As an Oncology Nurse - Transitions of Care, you will conduct clinical assessments, monitor for changes in health, coordinate care, and educate members and caregivers about their diagnosis and treatment over the phone. You will demonstrate a strong clinical focus and support the need for culturally competent care.
Responsibilities:
Conduct clinical assessments and monitor for changes in health.
Coordinate care, including transitions, and educate members and caregivers about their diagnosis and treatment.
Demonstrate a strong clinical focus and support the need for culturally competent care.
Help improve Thyme Care's service offerings by communicating feedback from members and providers to our clinical leadership.
Assist with other administrative projects as needed.
Requirements:
BSN and unrestricted Registered Nurse (RN) license.
At least 5 years of nursing experience with 3 years of oncology nursing or case management experience.
Certified Case Manager (CCM) certification.
Strong clinical focus and ability to work under pressure.
Effective listener and communicator with ability to build rapport and great working relationships.
