Position Overview:
JFS is looking for a compassionate, mission-driven individual to join the Older Adult Case Management (OACM) team as a Case Manager. In this role, you have the opportunity to use your resource navigating skills and experience working with older adults to make a meaningful difference in your community and help change lives across San Diego. The Case Manager plays an integral role on the team and supports the entire agency by connecting older adults to related resources and helping them navigate health care and social services. This position reports directly to the Older Adult Program Manager and works closely with the OACM team and the rest of Nutrition and Aging Care team.
Responsibilities:
Provide comprehensive assessments for individuals and families, identifying areas of need and helping clients set and achieve goals
Provide time-defined, goal oriented short-term case management
Monitor effectiveness of care plan and progress of goals
Attend to telephone calls, assessing each situation and screening for high-risk situations
Carry a caseload of individuals and families (estimated 50-60 households)
Work closely with other case managers to coordinate care for shared clients
Provide appropriate external referrals as needed and with a “warm hand-off”
Ensure clients are aware of public benefit programs for which they are eligible
Work with the client to formulate and execute realistic and time specific goals
Maintain accurate and detailed documentation of client interactions in software system with integrity and in line with the quality standard set by supervisor
Skills/Experience/Abilities That Are a Must-Have:
Bachelor’s degree in a human services field
Master of Social Work (MSW) preferred
Minimum of two years of experience in older adult case management/resource coordination role
Sensitivity to cultural differences present in the organizations/programs service population
Understanding of the low-income population and the concept of trauma informed care
Desire to work in an integrated team environment
Demonstrated persistence in finding resources that address the multi-factored impacts of poverty
Experience setting boundaries when working with stressed clients, often in some level of urgency
Ability to show an unconditional positive regard for clients who turn to JFS for support
Previous training in Motivational Interviewing preferred
Experience interacting with people living with dementia preferred
