Overview We are seeking a compassionate and experienced Palliative Medicine Nurse Practitioner (NP) to join our interdisciplinary care team. The NP will provide expert medical care to patients with serious, chronic, or life-limiting illnesses, focusing on symptom management, emotional support, and improving quality of life. This role involves collaboration with physicians, social workers, chaplains, and other healthcare professionals to deliver holistic, patient-centered care.
Responsibilities Conduct comprehensive assessments of patients’ physical, emotional, social, and spiritual needs.
Develop and implement individualized care plans in collaboration with the care team.
Manage medications and treatments to alleviate pain and other distressing symptoms.
Facilitate discussions around goals of care, advanced care planning, and end-of-life decisions.
Provide emotional and psychological support to patients and families.
Educate patients and families about disease progression, treatment options, and prognosis.
Participate in interdisciplinary team meetings and contribute to care planning.
Advocate for patients’ preferences and needs within the healthcare system.
Maintain accurate and timely documentation in electronic health records.
Stay current with best practices in palliative care through continuing education.
Qualifications Master’s Degree in Nursing (MSN) with NP certification.
Active NP license in the state of practice.
Certification in Hospice and Palliative Nursing (ACHPN) preferred.
Minimum of 2 years of clinical experience, preferably in palliative, hospice, or critical care.
Strong clinical assessment and diagnostic skills.
Excellent communication and interpersonal abilities.
Experience working in a multidisciplinary healthcare team.
Proficiency in patient education and counseling.
BLS and ACLS certification.
