Homebase Medical is getting back to basics. Our mission is to optimize the quality of life for homebound older adults, allowing them to live independently and with dignity in their own homes. We make house calls - enabling access to quality wrap-around services for homebound older adults with complex health needs while improving the quality of their lives, right at home. Homebase Medical is expanding rapidly into the Central LA, CA areas. We are currently searching for an experienced full-time Advanced Practice Clinicians (Physician Assistant or Nurse Practitioner) to join our expanding house call practice. You will be responsible for primary care in the home setting, Board and Care, and Assisted Living Facilities.
Key Duties
Conduct Annual Wellness Visits focused on patient education
Deliver Chronic Care Management to improve long-term outcomes
Perform comprehensive in-home visits including:
Medication review & chronic illness management
Medical history updates & physical exams
Labs, diagnostic screenings, and post-hospital follow-ups
Primary care management & palliative care services
Actively support the achievement of Homebase Medical's Vision and Goals.
Qualifications
Active Nurse Practitioner or Physician Assistant license in the State of California
Board certification (Nurse Practitioner or Physician Assistant)
DEA licensure
Prior experience preferred (but passion and commitment are just as important)

Typical Annual Earnings: $132,000 - $212,000+
Guaranteed Annual Base Salary: $68,640
Per-patient rate: $90 for every patient seen
Earning security: Each pay period, you’re paid whichever is higher — your base salary or your per-patient total
$500/month no-show protection
Mileage reimbursement per IRS guidelines
