Nurse Case Manager – Wound Care Registered Nurse Join Your Health as a Nurse Case Manager in the Savannah, GA / Hilton Head, SC service area. In this full‑time, salary‑based role you will travel to patients’ homes and long‑term care facilities to provide wound care, care coordination, and telemedicine visits. You must be a licensed RN with reliable transportation and the ability to work flexible hours.
Area of Responsibility Facilitate receipt of all medical records from primary providers and specialists.
Review patient records, complete consents, and enroll eligible patients in Care Management.
Initiate & document Care Management plans and capture diagnoses with high specificity.
Complete annual wellness visits, cognitive impairment screenings, social determinants of health assessments, and advance care planning discussions.
Evaluate patients for home health, hospice, palliative care, RPM devices, adaptive equipment, and safe environment needs.
Communicate and coordinate care with interdisciplinary teams and reconcile medications.
Provide post‑discharge education and maintain up‑to‑date care plans.
Qualifications Licensed RN in good standing (registration preferred). Minimum of 3 years of clinical experience.
Experience in community settings preferred.
Strong written and verbal communication, computer literacy, and teamwork skills.
Ability to travel between offices, facilities, and patients’ homes; licensed driver with a valid, insured vehicle.
Flexible schedule and willingness to travel across the Savannah/Hilton Head service area.
