Overview Nurse Case Manager - Trauma Clinic at Children's Wisconsin. The person in this role will work in a new role for the new clinic: Trauma Quality of Life Clinic. Utilizing the case management process, the person in this position is responsible for providing care coordination services to a group of complex, special needs pediatric patients and their families across the continuum of care. The case manager assesses, plans, implements, coordinates and evaluates the plan of care in partnership with the family and other members of the health care team.

Qualifications Education: Bachelor\'s Degree BSN (or equivalent knowledge) preferred
Experience: Knowledge of complex, chronic pediatric patients generally acquired through 2-3 years of pediatric experience
Licenses and Certifications: Current license to practice professional nursing in the State of Wisconsin is required; Current CPR certification - Basic Life Support (BLS) for Healthcare Providers
Certification as a Case Manager and experience in case management strongly desired
Schedule Hours: 7:30 am - 4:00 pm (M-F)
Full time: 40 hours per week
No weekends
Onsite: Milwaukee main campus
