Nurse Case Manager - Registered Nurse- Part Time We are seeking a Registered Nurse to service our patients throughout the Brunswick area. The Nurse Case Manager plays a critical role in the patients care team. The nurse will visit patients at home, long?term care facilities, and other sites to evaluate patients, facilitate provider visits, and initiate telemedicine encounters. Provide quality healthcare in adherence to all applicable laws, regulations, and policies within the scope of practice.

Key Responsibilities Facilitate receiving and reviewing all medical records from the patients primary provider and specialists.
Complete consents and enroll patients in Care Management if eligible.
Initiate a Care Management Plan of Care and capture all diagnoses at the highest specificity, ensuring they are accepted.
Perform Comprehensive Annual Wellness Visits (C?AWVs) and cognitive impairment screenings.
Conduct Social Determinants of Health (SDoH) assessments and advance care planning (ACP) reviews.
Evaluate eligibility for home health, hospice, palliative, or specialty consults.
Assess and recommend RPM devices, adaptive equipment, and durable medical equipment (DME).
Reconcile prescribed and OTC medications, supplements, and other treatments.
Provide post?discharge education and evaluate safe environment for patients.
Maintain and update Care Management care plans, and document all care management activities accurately.
Qualifications Must be a licensed Registered Nurse in good standing with the appropriate board/issuer.
Minimum of three (3) years of clinical experience preferred.
Experience in community settings preferred.
Proven ability to communicate effectively with interdisciplinary care teams, patients, and caregivers.
Strong written and verbal communication skills.
Basic computer knowledge and ability to maintain accurate electronic documentation.
Demonstrated leadership skills and ability to foster a cooperative work environment.
Team player capable of managing multiple responsibilities with sound judgment.
Flexible work hours and ability to travel between offices, facilities, and patient homes; licensed driver with an insured, reliable vehicle.
