We are seeking a Registered Nurse to service our patients throughout the Lexington, SC area. The role of the Nurse Case Manager is a critical part of the patient’s care team. The nurse will visit patients and facilitate appropriate provider interactions. The Nurse Case Manager will be required to travel to patients’ homes, long‑term care facilities, and other sites to evaluate patients and initiate telemedicine visits. Provide quality healthcare in adherence to all applicable laws, regulations, and policies within the scope of practice.
Travel & Schedule This is a full‑time, salary‑based position working 8‑hour shifts Monday‑Friday (8 am‑5 pm). Daily travel is required; a reliable driver with an insured vehicle in good working order is essential. Service area: Lexington, 29072.

What Are We Looking For?
Facilitate receiving all medical records from the patient’s primary provider and specialists.
Review medical records and complete consents with patients.
Enroll patients in Care Management when eligible.
Initiate a Care Management Plan of Care.
Capture all diagnoses at the highest specificity and ensure acceptance.
Complete AWVs to review by the provider.
Complete cognitive impairment screenings, SDoH assessments, and ACPs with patient guidance.
Evaluate referrals for home health, hospice, palliative care, RPM devices, and other resources that improve quality of life.
Reconcile prescribed and OTC medications, supplements, and herbal remedies.
Provide post‑discharge education and evaluate needs for adaptive equipment, DME, and safe environment.
Document and log Care Management activities accurately.
Qualifications
Registered Nurse licensed in good standing with the appropriate board.
Minimum of three (3) years clinical experience, preferably in community settings.
Ability to communicate and collaborate effectively with interdisciplinary care teams, patients, and caregivers.
Strong written and verbal communication skills.
Basic computer proficiency.
Leadership skills and ability to manage multiple responsibilities.
Flexible schedule and travel between offices, facilities, and patient homes.
Licensed driver with a vehicle insured in accordance with state or organizational requirements.
