We are seeking a Field Nurse Case Manager to cover the Northern Bay Area, CA, and surrounding areas . This is a home-based position with travel, up to 2 hours one-way, to doctors offices, hospitals, and other locations. The individual is responsible for medical case management of work-related injuries, including assessment, planning, coordination, implementation, and evaluation of injured/disabled individuals. The Case Manager collaborates with insurance carriers, medical providers, attorneys, employers, and employees, monitoring the injured workers progress and reporting results to the employer and insurance carriers.

Qualifications:
Current, unencumbered Registered Nurse (RN) license in California
Experience in workers compensation case management is preferred
Certifications (CCM, CRRN, CDMS, CRC) are strongly preferred and must be obtained within two years of hire
Skills: organization, adherence to timelines, PC and technology proficiency, effective communication (written, verbal, interpersonal)
Ability to work autonomously with minimal oversight
Patient education skills
Valid drivers license and good driving record
