Management of patient population to optimize utilization
Discharge planning
Coordinate with physicians, nurses, physical therapy and ancillary staff to facilitate all departmental processes.
Communicate with Home Health Agencies and Managed Care Plans, provide clinical information for admission and follow-up patient reviews.
Ensure payer documentation and information requirements are met.
Education and Requirements:
New York state License for Registered Professional Nurse and current registration or valid permit to practice in New York state as an R.N.
Bachelor's degree required
Minimum of 2-4 years of experience required in an acute care setting
Knowledge of Medicare Regulations
Excellent interpersonal and communication skills
Experience with electronic health records
Case Management Certification preferred (CCM)
