Nurse Case Manager - Hospice Registered Nurse We are seeking a Registered Nurse to serve patients throughout the Beaufort area. The Nurse Case Manager is a critical part of the patient care team, visiting patients to facilitate appropriate provider care and initiating telemedicine visits. The role requires travel to patients homes, long-term care facilities, and other locations in the service area. The position is full?time, salary?based, and involves 12?hr or 8?hr shifts.

What Were Looking For Facilitate receiving all medical records from the patients primary provider and specialists.
Review medical records.
Complete consents with patients.
Enroll patients in Care Management if eligible.
Initiate a Care Management Plan of Care if patient is eligible.
Capture all diagnoses at the highest specificity and ensure acceptance.
Complete AWVs to be reviewed by the provider.
Complete cognitive impairment screenings.
Complete social determinants of health (SDoH) assessments and/or screenings.
Complete ACPs to be reviewed with the patient by the provider.
Evaluate for home health, hospice, palliative, or specialist consults.
Evaluate for RPM devices, resources, or tools that may improve quality of life.
Communicate and coordinate care.
Reconcile prescribed and OTC medications, vitamins, supplements, herbal remedies, and other treatments.
Provide post?discharge education.
Evaluate for adaptive equipment and DME.
Evaluate for safe environment.
Evaluate acute condition(s) or follow?up from previous visits.
Accurately document and log Care Management activities; keep care plans up to date.
Coordinate with the patients health care team, providers, PT/OT, home health/hospice representatives, and others.
Facilitate visits with appropriate provider or entity.
Coordinate telehealth visits when necessary.
Coordinate with hospice interdisciplinary team and other individuals in the patients care plan.
Participate in IDG meetings, when necessary.

Qualifications Registered Nurse with a license in good standing.
Minimum of three (3) years of clinical experience preferred.
Experience in community settings preferred.
Strong written and verbal communication skills.
Proficient in basic computer use.
Effective interdisciplinary teamwork and collaboration.
Flexible hours and travel between offices, facilities, and sites.
Licensed driver with a valid automobile; vehicle must be insured and in good working order.
Ability to manage multiple responsibilities and make sound judgments.
Demonstrated leadership and teamwork skills.
Strong interpersonal skills under all conditions.
