What Youll Do

• Provide ongoing in-home assessments & nursing care
• Develop & manage individualized care plans
• Educate and support patients & caregivers
• Collaborate with healthcare providers to ensure coordinated care
• Maintain accurate EMR documentation & ensure compliance
• Participate in quality improvement initiatives

What Were Looking For

• Current RN license in Ohio (required)
• 1 year acute care or med-surg experience (home health preferred)
• Proficiency with EMR & Microsoft Office
• Valid drivers license & reliable transportation
