Position Overview:
The MyCare Ohio Case Manager assesses and manages care for clients aged 60+ enrolled in waiver programs. A Registered Nurse (RN) license and at least one year of experience with individuals with disabilities in a healthcare setting are required. The role involves a mix of home and community-based case management.
Qualifications and Responsibilities:
Current RN licensure in Ohio
Experience with individuals with disabilities
Strong assessment and communication skills
Knowledge of chronic illnesses, aging, pediatrics, and young adults
Effective communication skills, both in person and by phone
Ability to work collaboratively and maintain good relationships
Basic medical terminology and local resources knowledge
Computer skills, including Microsoft Office
Ability to lift/carry 20 pounds and have a vehicle available during work hours
