The Nurse Care Manager (NCM) Lead serves as a frontline leader and resource for Bloom Healthcares nursing care management team. This role provides day-to-day guidance, operational support, and mentorship to ensure high-quality, coordinated care for our patients. Working closely with the Clinical Support Manager, the NCM Lead helps drive team performance, supports ongoing education and process improvement, and fosters a positive, engaged team culture.

Education & Licensure: Active, LPN or RN license required.
Experience: Minimum 3 years of nursing experience in primary care, home health, or care management; prior leadership or mentorship experience preferred.

The pay range for this role in the state of Colorado typically falls between LPN $28-32 hourly / RN $39-44 hourly with the potential for performance-based bonuses and other benefits.
