Work Arrangement
- Training: Conducted on-site in the Brooklyn Office
- Hybrid Schedule: 4 days remote, 1 day field-based (Brooklyn area)
We are seeking a compassionate Registered Nurse to join a growing Managed Long-Term Care (MLTC) team. In this role, you will coordinate care, conduct in-home assessments, and support older adults in living safely and independently.
Key Responsibilities
- Conduct comprehensive in-home assessments and reassessments of members’ health and functional status.
- Develop and maintain individualized care plans tailored to each member’s needs.
- Coordinate and authorize covered services across providers and payers.
- Communicate and collaborate with members, caregivers, and interdisciplinary teams to ensure optimal outcomes.
- Serve as a clinical advocate, promoting member well-being and continuity of care.
- Maintain accurate documentation and compliance with MLTC guidelines and policies.
Requirements:
- Active NYS RN license & BSN
- Minimum 3 years RN experience; MLTC or geriatrics experience preferred

- Salary: $105K–$115K + $20,000 sign-on bonus
