Responsibilities Identify, engage, and enroll high-risk patients into the Complex Case Management program using provided risk lists
Perform comprehensive assessments of patients medical, social, psychological, and environmental needs
Develop and implement personalized, culturally competent care plans in partnership with patients, caregivers, and providers
Collaborate with a multidisciplinary team including PCPs, pharmacists, behavioral health specialists, and community health workers
Provide education, support, and coaching for chronic disease self-management and health promotion
Coordinate transitions of care and reduce unnecessary readmissions by communicating with hospitals and discharge planners
Monitor labs, medications, and clinical outcomes to ensure alignment with care guidelines and PCP plans
Conduct home visits and in-office face-to-face visits as needed to engage and support patients
Refer and connect patients to community-based services, and monitor progress to avoid service duplication
Participate in quality improvement initiatives, care conferences, and clinical program development
Who You Are (Qualifications) Licensed RN in the State of Rhode Island (required)
35 years of experience in community health, public health, or chronic disease management (required)
At least 2 years of experience managing patients with complex medical and social needs (required)
Strong background in case management and interdisciplinary care team collaboration
Certified in diabetes education or other chronic disease area, or willing to obtain within 12 months (preferred)
Bilingual (Spanish) is a strong asset (preferred)
Comfortable with home visits and travel between care sites; valid drivers license required
Excellent organizational, critical thinking, and communication skills
Proficient in EMRs and Microsoft Office Suite; comfortable using web-based health tools

Location Cranston, Rhode Island Role is primarily office-based with regular travel to partner practices and patient homes across the state.
Physical Demands Prolonged periods sitting at a desk or using a computer
Regular walking and standing, including entering healthcare buildings and patient homes
Workspace environments may vary across provider offices and may include stairs without elevators
Must be able to lift up to 15 pounds occasionally
Must be able to travel reliably to and from multiple locations
Salary range: $80,000 USD - $90,000 USD
