As a Nurse Care Manager, you will play a vital role in care coordination, including in-home assessments, planning, facilitation, advocacy, and authorization of covered services to meet members' health needs while promoting quality, cost-effective outcomes.
Key Responsibilities:
- Collaborate with members and their families to maintain the most independent living situation possible.
- Ensure consistent care across the healthcare continuum by assessing and closely monitoring members' needs and status.
- Authorize covered services and coordinate care across different payers.
- Communicate and collaborate with members, family members, caregivers, primary care practitioners, and the interdisciplinary team.
Qualifications:
- Current NYS RN License and registration
- Associateās Degree in Nursing Required. Bachelor of Science in Nursing (BSN) Preferred.
- Minimum of 3 years of RN experience
- Clinical expertise in geriatrics preferred; managed long-term care (MLTC) experience is required.
- Bilingual Cantonese, Mandarin, Spanish or Russian
Compensation:
- Annual salary: $105,000 - $115,000
- Eligible for a 20,000 sign-on bonus ($5,000 payable every six months, less taxes)
