Job Description
A Nurse Care Coordinator will oversee coordinated patient flow through inpatient stays, achieving safe, timely discharges. This role works closely with the interdisciplinary team, ensuring all disciplines are aligned on the patient’s plan of care.

Primary Job Responsibilities

Identify and monitor patients with high medical needs to ensure timely completion of consults, procedures, and tests, while maintaining ongoing team communication.

Lead medical team and nursing huddles, creatively removing barriers to discharge related to medical complications or system obstacles.

Assist with ordering complex medical equipment and ensure safe transitions of care.

Develop and monitor outcome measures related to care coordination, staff and patient satisfaction, and provide ongoing education on improving patient flow within the UWMC system.

Requirements

Master’s degree in Nursing or related field (applicable experience may substitute).

Three years of professional nursing experience.

Experience in Case Management.

Effective verbal and written English communication skills.

Strong interpersonal, leadership, and clinical expertise.

Proficient in accessing and documenting information in the electronic medical record.

Current Washington State RN license and BLS certification at time of hire.

Compensation, Benefits and Position Details
Pay Range Minimum: $43.82 per hour

Pay Range Maximum: $79.60 per hour

Other Compensation:

$4.50/hr Night Shift

$4.00/hr Weekend Shift

$2.50/hr Evening Shift

$1.00/hr BSN

$1.25/hr Nursing Certification
