Case Manager - Housing Navigator (Los Angeles, CA)
Position: Full Time, Monday thru Friday 8:30am to 5:00pm, Non-Exempt, Driving Required. Salary: $23.24 – $28.28 per hour. Pay Frequency: 24 Pay Periods Per Year. Lower end of range applies to candidates with minimum experience; higher end applies to seasoned candidates.

About The Job
As part of the Outreach Services team, the Case Manager‑Housing Navigator assists clients in breaking the cycle of homelessness through a “whatever it takes” approach. This position supports clients in all phases of homelessness—including street, interim housing, transitional housing, and permanent housing—helping them access healthcare, social services, support systems, and navigate coordinated entry. The role provides individualized supportive services, helping each client develop a plan to address strengths, barriers, health needs, income, and secure and maintain housing.
Position Responsibilities
Conduct intake and individualized needs assessment for all participants, collaborating to develop an Individualized Service Plan (ISP) addressing housing, income, and personal goals.
Provide referrals, linkages, information, and support to resources that help participants achieve ISP goals.
Evaluate strengths and challenges, conduct a 90‑day review with the participant after intake.
Maintain documentation standards, completing case file notes promptly and keeping an accurate participant file with required documents.
Maintain confidentiality of participant files per HIPAA and applicable guidelines.
Develop therapeutic and professional relationships that enhance engagement and retention of individuals with housing goals.
Provide crisis intervention and risk assessment services focused on safety and empowerment.
Use motivational interviewing and trauma‑informed care in service delivery.
Maintain current knowledge of community resources and utilize them to provide comprehensive, wrap‑around services.

Preferred Qualifications
Two (2) years of experience in healthcare, case management, homeless services, or related field working with vulnerable populations.
Minimum Qualifications
All levels of education and experience are welcome.
Mandatory Requirements
Employment eligibility verification.
CPR/First Aid training or willingness to obtain training.
Completion of tuberculosis test, background screening, and drug test as condition of hire.
Driving
Reliable transportation, valid driver’s license, proof of insurance and ownership for personal vehicle used during work duties, and ability to qualify for PATH’s insurance coverage.
Employment Eligibility
Applicants must be authorized to work in the U.S. ETA cannot sponsor or take over visa sponsorship. PATH does not employ individuals who are currently receiving services from PATH or have received services within the past two years.
California‑Based Work Expectations
All regular and customary work must be performed within California. The position does not offer fully remote work; schedules may involve multiple sites but not home work.
