Summary

The Coming Home program employs the Housing First approach to housing and providing supportive services to those experiencing chronic homelessness. Essential Responsibilities

Performs biopsychosocial assessments and partners with clients to create individualized, client-centered support plans focused on mental health and wellness.

Coordinates psychiatric care, therapy appointments, and medication access in collaboration with mental health providers.

Assists clients in obtaining and maintaining access to mainstream resources, including physical and mental health care, substance use treatment, public benefits, housing resources, and employment services.

Advocates for the needs of individual clients and identifies opportunities for systems-level improvements to support individuals with behavioral health challenges.

Navigates the Social Security, Medicaid, Medicare, and other benefits systems on behalf of and/or with clients to support income stability and access to care.

Maintains thorough, timely, and accurate documentation of all client interactions, assessments, referrals, and progress in client records and HMIS.

Monitors client progress through ongoing contact and regular follow-ups, maintaining collaboration with internal and external service providers.

Communicates all clinical assessments, interventions, and case management activities in the client record daily.

Collaborates with the Peer Support Specialist to deliver trauma-informed, recovery-oriented services and workshops that promote mental health literacy and reduce stigma.

Provides crisis intervention and support for clients experiencing acute mental health episodes or instability.

Participates in daily team meetings to coordinate client services, share updates, and ensure continuity of care across the program.

Attends and participates in clinical supervision, administrative meetings, and case conferences as required.

Coordinates with internal and external programs to facilitate referrals and service access, serving as a liaison between clients and community partners.

Assists with client moves, including assembling, picking up, delivering, and setting up household items to support successful housing placements.

Transports clients using a City of Amarillo vehicle to attend appointments, meetings, and services as needed.

Carries a program-issued cell phone and remains accessible to clients, partners, and supervisors while in the field.

Meets all program and funding documentation requirements, including Medicaid authorizations, HMIS data entry, and grant-related reports.

Contributes to creating a culture of support, community, and collaboration within the Coming Home team and the Community Development Department.

Performs other job- and department-related duties as assigned.

Minimum Requirements

A Bachelor's Degree in Social Work, Psychology, Counseling, or a related field is required. A combination of education and field experience may be considered instead of a degree. Must have a valid Texas drivers license.

Must possess or be able to obtain a SOAR (SSI/SSDI Outreach, Access, and Recovery) certification.

If not currently SOAR certified, the candidate must complete the SOAR online training and obtain certification within 6 months of hire.

Preferred Requirements

A Master's Degree in Social Work, Psychology, Counseling, or a related field is preferred. Two years of experience in providing case management or direct services to individuals with behavioral health needs, those experiencing homelessness, or other underserved populations is preferred.

Knowledge, Skills, and Abilities

Demonstrated ability to engage clients from diverse backgrounds with empathy and respect.

Strong knowledge of community mental health systems, trauma-informed care, and recovery principles.

Proficient in operating small office equipment, computers, and software for data entry and communication.

Excellent verbal and written communication skills.

Ability to work both independently and collaboratively within a multidisciplinary team.

Comfortable providing services in a variety of community-based settings, including encampments, shelters, and homes.

Flexible availability for occasional after-hours work to meet client needs or attend events.

Must be willing and able to drive a City of Amarillo vehicle as part of regular duties.

Work Environment

Will work Monday-Friday, 8 am to 5 pm. May require occasional after-hours work to attend meetings, finish projects, and meet deadlines.
