Position: Medicaid Care Manager
General Description:
The Medicaid Care Manager is responsible for providing services to individuals whom of which have been accepted into the program. These services may include providing general information about the program, developing comprehensive goal-oriented service plans, providing an appropriate level of guidance and support, facilitating referrals, and serve as an advocate on behalf of participant to assist them in securing services, entitlements and support to reach their goals.

Licensing/Certification Requirements: Valid NYS Driver’s License required.
Physical Demands: Must be able to climb multiple sets of stairs. Must be able to lift 20 pounds.
Salary: $45,000 yearly
