Pay Range
Base pay range: $29.00/hr - $38.75/hr. This range is provided by Sunrise Senior Living. Your actual pay will be based on your skills and experience – talk with your recruiter to learn more.

Job Description
The LPN Medication Care Manager is responsible for providing the highest degree of quality care and services by administering medication and treatments in a safe, organized manner. Responsibilities include but are not limited to administration of medications, documentation, conducting SHUs, and providing resident care while demonstrating the Mission for Sunrise Senior Living, “to champion the quality of life for all seniors” in accordance with federal, state/provincial, and local laws, standards and regulations, and Sunrise Senior Living policies.
Responsibilities & Qualifications
Medication Administration
Provide high quality care by administering medication and treatments safely.
Review, read, notate, and initial the electronic health record.
Receive medication updates from Resident Care Director (RCD) or Wellness Nurse.
Administer, assist with, and observe medications per the medication administration record and the Six Rights.
Ensure medications are passed on time using a mobile medication cart.
Document and initial as medications are given and record refusals or missed doses.
Maintain confidentiality of resident information and medications.
Report concerns or unavailable medication to the RCD or Wellness Nurse.
Restock medication cart after all medication passes.
Assist in checking medication regardless of packaging system.
Assess need for “as needed medication” and document/report to supervisor.
Count all narcotics with another Medication Care Manager or Lead Care Manager each shift and report discrepancies.
Maintain and clean the Medication Room, carts, and treatment carts.
Follow re-fill process for medications.
Help residents maintain independence and promote dignity and safety.
Respond to residents with empathy, respecting communication patterns.
Practice good standard care precautions.
Audit medication carts.
Resident Care
Notify RCD of any resident or family concerns.
Attend and actively participate in daily Cross Over meetings.
Conduct Service and Health Updates as directed by RCD.
Participate in developing Individualized Service Plans.
Transcribe orders.
Risk Management and General Safety
Partner with community team to ensure compliance with regulations.
Practice safety procedures: PPE, fire extinguishers, SDS, Lockout Tagout.
Report all accidents/incidents immediately.
Report unsafe and hazardous conditions immediately.
Keep cords, carts, equipment out of the way and fire code compliant.
Comply with infection control techniques.
Handle exposure to blood borne pathogens and hazardous materials.
Report occupational exposures immediately.
Ensure safe storage of oxygen tanks.
Training and Team Success
Participate as a team member and commit to team goals.
Demonstrate the Team Member Credo in daily interactions.
Serve residents and guests through Principles of Service.
Contribute to engagement programs and processes.
Attend regular meetings and training sessions.
May be designated as shift supervisor or supervise other managers.
Maintain compliance with required training and regulations.
Perform other duties as assigned.

Experience and Qualifications
LPN, LVN, or state/province licensed nurse credential.
Maintain required certifications: CPR, First Aid.
Must be at least 18 years of age.
Previous experience working with seniors preferred.
Desire to serve and care for seniors.
Driver qualifications if applicable must be signed.
