Job Overview The LPN Medication Care Manager is responsible for providing the highest degree of quality care and services by administering medication and treatments in a safe organized manner. Responsibilities include but are not limited to administration of medications, documentation of medication administration, conducting SHUs, and providing resident care while demonstrating the Mission for Sunrise Senior Living, “to champion the quality of life for all seniors” in accordance with federal, state/provincial, and local laws, standards and regulations and Sunrise Senior Living policies to promote the highest degree of quality care and services to our residents.
Responsibilities & Qualifications Essential Duties include:
Medication Administration: Provide the highest degree of quality care and services by administering medication and treatments in a safe organized manner.
Resident Care: Notify RCD of any resident and/or family concerns, attend and actively participate in daily Cross Over meetings, conduct Service and Health Updates as directed by RCD, and participate in the development of the Individualized Service Plans (ISP).
Risk Management and General Safety: Partner with community team to ensure community is in compliance with national/provincial regulations, practice safety procedures, and report all accidents/incidents immediately.
Training and Contributing to Team Success: Participate as a member of a team, commit to working toward team goals, demonstrate in daily interactions with others, our Team Member Credo, and contribute to the overall engagement programs and processes.
Core Competencies Ability to make choices and decisions and act in the resident’s best interest
Ability to react and remain calm in difficult situations
Ability to handle multiple priorities
Possess written and verbal skills for effective communication and level of understanding
Demonstrate good judgment, problem solving and decision-making skills
Experience and Qualifications LPN, LVN, or state/province specific licensed nurse credential
Maintain certifications, including CPR and First Aid
Must be at least 18 years of age
Previous experience working with seniors preferred
Desire to serve and care for seniors
