Base pay: $26.60/hr – $35.55/hr (actual pay based on skills and experience)

Job Overview
The LPN Medication Care Manager is responsible for providing the highest degree of quality care and services by administering medication and treatments in a safe, organized manner. Responsibilities include but are not limited to administration of medications, documentation of medication administration, conducting SHUs, and providing resident care while demonstrating Sunrise Senior Living’s mission to champion the quality of life for all seniors.

Responsibilities

Administer medications and treatments safely, observing the Six Rights of Medication Pass.

Document medication administration and maintain accurate electronic health records.

Receive and respond to medication updates from the Resident Care Director or Wellness Nurse.

Maintain medication cart, restock, audit, and ensure medications are dispensed on time.

Count narcotics with another Medication Care Manager or Lead Care Manager each shift.

Manage medication room cleanliness and keep track of expired medications.

Assist residents in medication independence and uphold dignity and safety.

Notify RCD of resident concerns, medication issues, and lost or unavailable medication.

Attend daily Cross Over meetings and Service and Health Updates.

Contribute to the development and review of Individualized Service Plans (ISP).

Participate in risk management and compliance with safety protocols, including PPE, fire safety, and infection control.

Report all accidents, incidents, and unsafe conditions immediately.

Maintain lockout–tagout and oxy‑tank safety procedures.

Engage with team members, participate in training, and may serve as shift supervisor or supervise other MCMs.

Perform additional duties as assigned.

Qualifications

LPN or LVN credential (state/province specific) with all required certifications.

CPR and First Aid certification.

Minimum 18 years of age; preferred experience with seniors.

Strong written and verbal communication skills.

Demonstrated judgment, problem‑solving, and the ability to handle multiple priorities.

Ability to act in the resident’s best interest and maintain confidentiality.

Competence in medication administration, documentation, and safety protocols.
