Overview
The LPN Medication Care Manager is responsible for providing high-quality care by administering medications and treatments in a safe, organized manner. The role supports Sunrise Senior Livings Mission to champion the quality of life for all seniors and complies with federal, state/provincial and local laws, standards, and Sunrise policies.
Responsibilities
- Medication Administration: administer, assist with, and observe medications and treatments; document administration; maintain the Medication Room and carts; follow the Six Rights of Medication Pass; ensure timely medication passes; restock medications; count narcotics with another MCM/Lead and report discrepancies.
- Documentation: review, notate, and initial the electronic health record; document refusals or missed doses; transcribe and communicate orders as required; maintain confidentiality of resident information.
- Resident Care: notify RCD of concerns; participate in daily meetings; contribute to the development of Individualized Service Plans (ISP); transcribe orders.
- Safety and Compliance: follow infection control techniques; practice PPE use; report accidents/incidents and unsafe conditions; ensure hazardous cords and equipment are managed in compliance with fire codes.
- Training and Teamwork: participate in team goals; attend regular trainings and meetings; may be designated shift supervisor or supervise other MCMs; maintain required trainings and certifications.
- Core Competencies: demonstrate sound judgment, effective communication, and the ability to manage multiple priorities while acting in residents' best interests.
Qualifications
- LPN, LVN, or state/province licensed nurse credential ( CPR and First Aid as applicable );
- Must be at least 18 years of age;
- Previous experience working with seniors preferred;
- In jurisdictions where required, maintain applicable certifications and driver requirements if applicable.
Benefits and Compensation
- Base pay range: $29.00/hr - $38.75/hr; Sunrise-provided pay range information and the understanding that actual pay is based on skills and experience.
