LPN LVN - Utilization Management
St. Joseph Home Health in Anaheim, CA is hiring Licensed Practical or Vocational Nurses (LPN/LVN) for a utilization management role. The position is open for a full 1.0‑FTE (40 hrs/week) day shift and a part‑time 0.4‑FTE (16 hrs/week) day shift. This is a hybrid role: 20% onsite in Anaheim and 80% remote.
Key Responsibilities
Advocate for patients to achieve optimal health, access to care and appropriate resource utilization.
Collect data, assist in planning, implementation, coordination, monitoring and evaluation of the plan of care.
Manage resources and coordinate the continuum of care in accordance with St. Joseph Health policies.
Required Qualifications
Associate’s Degree in healthcare or high school diploma/GED with graduation from an accredited nursing program and 2 years of related work experience.
California Licensed Vocational Nurse upon hire.
2 years clinical experience in an HMO, medical group, affiliated model, hospital or medical office/clinic setting.
Preferred Qualifications
Bachelor’s Degree in a healthcare-related field.
3 years experience in utilization management or case management.
Benefits & Compensation
The base pay range is $39.04 to $60.60 per hour. Additional compensation may include shift differentials, standby/on‑call pay, overtime, premiums, extra shift incentives or bonuses.
