Overview
This position assesses eligibility for long‑term support through IDD, provides initial education and community resources, develops state plans, and transfers new cases to Case Manager 2 for further coordination and case management. The role may last up to two years and can end at any time.
Responsibilities
Review eligibility for long‑term support through IDD for new clients.
Distribute resource packet to families and individuals based on eligibility.
Schedule and complete initial choice‑advising meetings over phone or at client homes per Medicaid plan eligibility.
Complete state plan and conduct site visits to ensure homes are safe and free from abuse.
Review eligibility for OHP and provide information for presumptive Medicaid disability determination.
Refer individuals for Oregon needs assessment after Medicaid determination.
Maintain a caseload of unfunded plans and refer funding requests to lead Case Manager 2.
Document visits, meetings, and discussions with eligible individuals.
File all initial paperwork, including state plans, return calls, and emails.
Provide consultation to co‑workers and partner program or agency staff.
Minimum Qualifications / Transferable Skills
Demonstrated ability to promote a culture of respect, inclusiveness, and appreciation of diverse perspectives.
High school diploma or GED.
At least 2 years of paraprofessional‑level experience providing services to human services/social service clients, including individuals with Intellectual Developmental Disabilities.
Must pass a criminal background check.

Additional Information
Telework: This position is designated as routine telework, with the possibility of occasional office visits. Remote work requires residence in Oregon or Washington. The onsite location is Five Oak, 409 SW 4th, Portland, OR 97209.
