Job Summary
The Lead Onsite Inpatient Case Manager assists the Managers and Director in managing the overall operations of the Inpatient case management department for Kaiser Permanente Core Facilities in accordance with current Federal and State regulations and guidelines. The Lead Onsite Inpatient Case Manager works in collaboration with the Managers, Director, and Sr. Director of Inpatient Case Management to manage and implement the strategic plan for the Inpatient Case Management department as assigned. The Lead Onsite Inpatient Case Manager collaborates with the Hospitalists and CWD physician leaders. The Lead Onsite Inpatient Case Manager reports to the Manager of Inpatient Case Management. The Lead Onsite Inpatient Case Manager has frequent contact with the support staff, QRM staff, CWD physicians, Hospitalists, Core staff, Directors, Sr. Directors, contracted providers, community resources, patients, and families.
Effectively manages the activities of the care coordination team, providing leadership and guidance.
Has authority to recommend staffing plans, interview, recommend for hire, coach, and evaluate staff.
Acts as a resource for the onsite ICM team, physicians, health plan administrators, and contracted vendors.
Assists in problem solving for difficult cases.
Works with leadership to identify goals, develop action plans, and communicate effectively with staff.
Works directly with staff to move patients through care continuum, monitor, coach, intervene, and ensure safe, appropriate discharge plans.
Collaborates with providers to ensure smooth transitions of care, aiming to reduce readmissions and improve outcomes.
Maintains proficiency in Kaiser Permanente's utilization management and discharge planning processes, providing training and education.
Identifies training needs, develops, and participates in staff training and onboarding.
Participates in on-call coverage, providing after-hours and weekend support.
May attend key operational meetings and float between facilities as needed.
Manages reports as delegated, demonstrates leadership behaviors, and participates as a hands-on team member.
Maintains professional growth and development.
Experience
Minimum two (2) years in utilization management, case management, or discharge planning in an acute care setting.
Minimum two (2) years of current clinical nursing experience in an acute care setting.
Education
Bachelor's Degree in Nursing required.
License, Certification, Registration
Registered Professional Nurse License (Georgia)
Additional Requirements
Knowledge of ACMA, CMSA, and NASW Standards of Practice.
Knowledge of healthcare and community resources.
Broad knowledge of healthcare delivery and case management within a managed care environment.
Understanding of laws, regulations, and standards affecting case management practice.
Strong critical thinking, problem resolution, and customer service skills.
Proficient in computer systems.
Preferred Qualifications
Supervisory/lead experience in UM/Case Management in a hospital setting.
CCM/ANCC Certification or equivalent preferred.
Experience in team resource planning and organization.
Inpatient care or health education experience.
Leadership experience in acute case management.
Master's in Nursing or Case Management preferred.
