Pay Range: Up to $80,000 + $3,000 Sign on Bonus

The Position
We are looking for a full-time Lead Clinical Care Manager with experience in a home-care setting, office-healthcare, care coordination, team development and/or clinical education. The Lead Clinical Care Manager is an expert in the duties of the Clinical Care Managers and leads the team to clinical success. Duties include, but are not limited to:

Serves as collaborator to Clinical Care Manager team and Clinical Leadership.

Clinical subject matter expert and preceptor for new Clinical Care Managers

Manages Clinical Care Manager resources to meet regulatory and compliance deadlines for assessments, authorizations, orders, infection reporting and incident reporting.

Responsible for daily monitoring and quality metric reporting.

Assumes caseload of clients when Clinical Care Managers are unable to manage.

Effectively manages a modified caseload of clients when necessary.

Conducts assessments, intakes as needed to support the team.

Leads 1 on 1 meetings with assigned team members.

Ensures all direct care provided by staff is compliant.

Provides training and guidance to agency direct care staff regarding orders for disciplines and/or treatments and services, consistent with agency policy.

Ensures referrals result in a timely start of care or are well documented for permitted delays or non-admissions.

Completes all supervisory visits, comprehensive assessment/reassessment visits per Agency policy, federal and state rules, and documents the visits.

Coordinates care between direct care staff and external providers.

Serves as liaison between the Agency and physicians, hospitals, and other agencies regarding patient care.

Provides a positive attitude and example of professional behavior for direct care staff.

Provides orientation and competency verification of direct care staff.

Provides direct, in-field supervision of newly hired clinicians and assures supervision of all staff is completed per regulations and agency policy including documentation.

Provides direct response and guidance to staff for problem resolution in patient care.

Provides information and reports as instructed.

Participates, as requested, in orientation of new staff, in-service education, continuing education, utilization review and QAPI programs.

Participates in/attends staff development activities and in-service meetings to educate the personnel.

Provides training and in-services to agency direct care staff on topics required by state licensure, clinical services, rules and regulations, agency policies and expectations.

Are You the Right Candidate?

Current Registered Nurse (RN) in good standing and currently licensed

Graduate of an accredited school of nursing

Home Health, pediatric, and/or office experience a plus!
