Lead Care Coordinator

As the Lead Care Coordinator, you will take a leadership role in guiding care coordination strategies across multiple practices. You'll oversee initiatives that align with CMS, ACO, and payor requirements, mentor team members, and ensure quality and performance goals are met. Responsibilities

Essential Duties and Responsibilities (includes, but not limited to):

  • Lead and mentor care coordinators, providing guidance, training, and support to ensure consistent execution of care coordination strategies.
  • Serve as the primary liaison between interdisciplinary teams, practice managers, and leadership to drive organizational goals related to quality, care coordination, and financial performance.
  • Develop and manage in-person and virtual meetings with providers and staff to communicate results, share best practices, and identify improvement opportunities.
  • Monitor compliance with payor and regulatory requirements and proactively address gaps.
  • Prepare and distribute meeting documentation and performance reports.
  • Act as a patient advocate and champion for quality improvement initiatives.
  • Perform additional leadership duties as assigned by the supervisor.

Qualifications

Before we go any further, we do have some deal-breakers. You must have:

  • Current state Registered Nurse or LPN license as required by location.
  • Registered Nurse experienceTwo years of experience with adult medicine and pediatric patients in primary care/ambulatory care, home health agency, skilled nursing facility, or hospital medical-surgical setting, within the past three years.
  • LPN experiencea minimum of five years' primary care practice experience with adult medicine and pediatric patients in a primary care/family practice setting.
  • Care management experience preferred.
