MedStar Health is looking for an LCSW-C/LICSW Inpatient Case Manager to join our team at MedStar Washington Hospital Center! The LCSW-C/LICSW serves as a member of the Case Management Team and facilitates the delivery of quality, cost‑effective, patient‑centered care from pre‑admission through post‑discharge. The role ensures that care is designed to meet individualized patient outcomes, monitors services delivered to selected patient populations during the acute hospital stay, works collaboratively with multidisciplinary teams both internal and external to the organization, promotes effective case management and resource utilization, and strives to achieve optimal clinical and resource outcomes for both acute and post‑hospital phases.

Primary Duties
Collaborate with the multidisciplinary healthcare team to assess patient needs, develop and coordinate individualized plans of care, and facilitate safe, timely discharges.
Conduct psychosocial and socioeconomic assessments, provide clinical evaluations for behavioral health concerns, and connect patients with appropriate community and social service resources.
Facilitate communication between patients, families, and healthcare providers to address social, emotional, cultural, and financial barriers to care.
Document assessments, referrals, and interventions accurately while maintaining compliance with departmental policies, quality standards, and regulatory requirements.
Participate in quality improvement initiatives, meetings, and professional development to enhance care management effectiveness and departmental performance.
Qualifications
Master’s degree in social work from a school accredited by the Council on Social Work education (Required).
1‑2 years of experience in social work, preferably in a hospital setting (Preferred).
1‑2 years of experience in care/case management (Preferred).
License and Certifications
LCSW-C – Licensed Certified Social Worker – Clinical License in the District of Columbia or the State of Maryland depending on work location (Required).
LICSW – Licensed Independent Clinical Social Worker License in the District of Columbia or the State of Maryland depending on work location (Preferred).
CCM – Certified Case Manager (Preferred).
Hiring Range
USD $74,214.00 – $134,596.00 per year.
