Key Responsibilities Conduct comprehensive physical, emotional, and psychosocial assessments, developing individualized care plans in collaboration with our interdisciplinary team.
Provide expert pain and symptom management while supporting the unique needs and goals of each patient.
Work closely with physicians, social workers, chaplains, and caregivers to ensure seamless, coordinated care.
Offer compassionate education and emotional support to patients and families, empowering them to make informed decisions about end-of-life care.
Maintain timely and accurate documentation in electronic medical records (EMR) in compliance with regulatory requirements.
Participate in interdisciplinary team meetings, contributing insights that enhance patient-centered care.
Serve as a resource for patients, families, and healthcare providers on pain management, symptom control, and hospice philosophy.
Engage in ongoing education and professional development to uphold the highest standards of hospice care.
Support Unified Care Hospice’s growth by referring qualified professionals who align with our values and mission.
Qualifications Current Registered Nurse (RN) license in the state of Texas (required).
Hospice or home health experience is preferred, but not required—we provide training and mentorship.
Strong communication and interpersonal skills, with a deep sense of empathy and connection to patients and families.
Proficiency in electronic medical records (EMR) systems and documentation.
Ability to collaborate effectively within an interdisciplinary team.
Reliable transportation for patient visits (mileage reimbursement provided).
Comfortable with travel and a flexible schedule.
COVID-19 vaccination required.
Schedule & Availability Day, Evening, Night, or Overnight shifts available.
Monday–Friday with rotating on-call responsibilities.
Additional hours may be available on holidays or as needed.
