Johns Hopkins Intrastaff is the internal staffing agency for the Johns Hopkins Health System and partner hospitals, providing temporary support to a variety of Johns Hopkins locations.

Responsibilities In collaboration with patients/families, social workers, physicians and the interdisciplinary team, provide leadership and advocacy in coordinating patient-centered care across the continuum to facilitate optimal transitions and progression in care.
Manage clinical resources and transition planning for patients within an assigned caseload from pre-admission through post-discharge; identify and eliminate barriers to delivery of clinical services with the patient, family and care team.
Develop a sustainable, safe transition plan appropriate to patient needs and resources in collaboration with patient/family and the interdisciplinary care team.
Maintain knowledge of clinical system and fiscal data related to specific patient populations.
Complete initial screening of assigned patients to identify readmission risks, patient strengths and needs related to clinical resource utilization and transition planning; initiate, complete and document transition planning for patients in the caseload.
Collaborate with healthcare team to ensure the multidisciplinary plan of care is developed, followed, and modified as needed; coordinate delivery of clinical services to meet clinical, quality and cost outcomes.
Strategize with physicians, specialists and payors to develop appropriate care delivery strategies for assigned patients.
Function as a consultant to health care team members on clinical, research and system problems; educate physicians about managed care principles, discharge planning, reimbursement, levels of care and continuum of care.
Coordinate and lead multidisciplinary rounds.
Identify high-risk patient behaviors, high-risk diagnoses, and social determinants of health; work to decrease readmission risk while improving quality outcomes.
Collaborate with community-based case managers and communicate handoffs to providers to facilitate continuity of care.
Maintain appropriate documentation in the medical record and care management files.
Identify indicators and collect/evaluate data related to the case-managed population.
Lead development, evaluation and revision of critical paths; analyze variances and trends to improve clinical, quality and fiscal outcomes.
Attend and participate in relevant committee meetings (e.g., Joint Practice Committee, barrier meetings/long stay review, length of stay reduction work groups) and departmental change initiatives.
Educate patients/families to optimize independence in self-care and improve outcomes.
Other duties as assigned.
Qualifications Education: Completion of an accredited BSN Nursing Program (RN). Master of Science in Nursing (MSN) preferred.
Knowledge, Skills, and Abilities: Ability to communicate effectively both orally and in writing; demonstrate empathy and build positive, collaborative relationships with hospital staff, patients, families, post-acute providers and payers.
Strong problem-solving abilities with clinical judgment.
Ability to function effectively in a fluid, dynamic, and rapidly changing environment.
Business acumen to maximize health benefits and patient/provider satisfaction while ensuring program viability through cost-effective interventions.
Proficiency with electronic medical records, email, and general office tools.
Licensure, Certification, On-going Training: Valid Maryland Nursing License required.
ACM-RN (Accredited Case Manager – RN) or CCM (Certified Case Manager) preferred.
Work Experience: Minimum five to seven years of nursing experience.
Minimum of one year in case management, discharge planning, or utilization review.
Rate: $55/hr
